Leadership Camp Spots Still Open!

 2018 FFA Leadership Camp Registration- Spots still Available!

We still have spots available for 2018 Illinois FFA Leadership Camp! The cost of Leadership Camp is $200.  The FFA Leadership Camp will be held on July 24-27, 2018 at the 4-H Camp in Monticello, Illinois. 

 Please register all of your students, and send in payment to the Illinois FFA Center.  Please make checks payable to the Illinois FFA, and please send the invoice in with the check.

  • Hopper Funds - If you would like to use Hopper Funds, please contact Tammy Yard at  She will be giving you a Discount Code to be used at checkout on the Caboodle System.  If you are wanting to use Hopper and have an Alumni Scholarship, please mention that when speaking with Tammy.
  • Alumni Scholarship Winners/Chapter Baskets - Please see Patti Davis' email for the Discount Code that the FFA Chapter will need to use at checkout to receive the amount of the scholarship(s) off of your total cost of camp.  
  • Cancellations for campers must be received by July 15, 2018, after this date the FFA Chapter/Camper will be liable for the registration cost. 
  • First Time Sign-Up for both FFA Advisors/Parent/Guardian:  The first/last name on the sign-in screen is what will be on your invoice, so please follow these instructions (this information is also located on the attached instructions for both FFA Advisors/Parents):
    • FFA Advisors - First Name is your School's Name (Ex. Athens) Last Name is your name (Ex. Mike Butler).  (If you are leaving your school, please give this information to the new teacher.)
    • Parent/Guardian - The First/Last Name is the Camper's First/Last Name (not the parent's name).
  • FFA Advisors - You will need to register your total number of male/females for camp (we are not collecting names until the Health Profile is completed by the parent).   You will also need to help each parent complete the Health Profile on their child that is attending leadership camp through the Caboodle Registration System (we are not utilizing CampDoc this year).  Health Profiles must be completed by July 1, 2018.
  • Parent-Guardian Instructions – Highlights:

  • Completed health profile is due by July 1st.
  • They need to sign-up into the system with the camper’s name – not the parent’s name. 
  • Please put the FFA Chapter’s Name when requested, not the section number.
  • The parent and camper must sign the Parent-Required-Upload to Cart-FFA Leadership Camp Signature Form-Insurance Card – we will not accept any other type of authorization form, and the back of the insurance card information must be written in the lines provided which include Telephone Number, Website, and email address.  Any pertinent information that the hospital/prompt care would need to charge your insurance for the visit.
  • Place the front of the insurance card at the bottom of the signed Signature Form-Insurance Card, and take a picture and send to your computer, or scan to your computer. 
  • The signed agreement form-insurance card must be uploaded before completing the health profile.  The button to upload the insurance card will appear after answering the survey questions, and before clicking “complete order”.  It is located in the middle of the screen and says “Attention Parents - Click this Button to upload the agreement form-insurance card”.  Upload the form, and then click “complete order”, once you complete the order, you will receive an email that we have received the information.
